Effect of a surgery-based comprehensive treatment on prognosis of stage IIIC cervical squamous carcinoma: A single center retrospective study.
Abstract
e17520 Background: Stage IIIC of cervical cancer is when lymph node metastasis occurs. Lymph node metastasis is one of the most important prognostic factors. The NCCN guidelines recommend concurrent chemoradiotherapy (CCRT) as the first choice of treatment, but the outcomes remain unsatisfactory. Based on the experience of our hospital, we propose the Obstetrics and Gynecology Hospital of Fudan University (FUOG) Treatment” as a surgical-based comprehensive treatment for stage IIIC cervical cancer. Methods: This study retrospectively collected data from patients with stage IIIC squamous cervical cancer who underwent FUOG Treatment in our hospital from January 2016 to December 2022. The FUOG Treatment refers to a treatment plan involving radical surgery for cervical cancer, followed by comprehensive adjuvant therapy such as CCRT and systemic chemotherapy based on the pathological results. The main outcomes were 5-year PFS and OS. Results: A total of 1326 cases were included in this study. The median follow-up time was 43 months. The 5-year OS was 82.0% for patients with stage IIIC squamous cervical cancer, 83.8% for stage IIIC1 and 61.6% for stage IIIC2, respectively. The 5-year PFS was 77.1% for all patients with stage IIIC squamous cervical cancer, 79.3% for stage IIIC1 and 52.4% for stage IIIC2, respectively. The 5-year OS and PFS of patients with stage IIIC1 was better than that of patients with stage IIIC2 (p=1.97e-06, 1.73e-07). Conclusions: Our study demonstrates that the FUOG Treatment for stage IIIC squamous cervical cancer, can significantly improve prognosis. The dissemination of this program is expected to have a profound impact on the prognosis of these patients.
