Neoadjuvant adebrelimab plus SOX and nab-paclitaxel in resectable locally advanced gastric and gastroesophageal junction adenocarcinoma: A multicentre, single-arm, prospective phase II trial.
Abstract
344 Background: We report preliminary results from an ongoing single-arm phase II clinical trial evaluating neoadjuvant adebrelimab combined with chemotherapy in patients with resectable locally advanced gastric or gastroesophageal junction (LA-G/GEJ) adenocarcinoma. Methods: All eligible patients will receive neoadjuvant treatment consisting of adebrelimab, SOX (S-1 plus oxaliplatin), and nab-paclitaxel every 21 days for four cycles. The primary endpoint is the pathological complete response (pCR) rate. Secondary endpoints include major pathological response (MPR), objective response rate (ORR), and others. Results: As of August 1, 2025, a total of 11 patients had signed informed consent and received neoadjuvant therapy. One patient discontinued treatment after one cycle of neoadjuvant therapy due to a severe rash. Among the 11 patients, there was 1 complete response, 5 partial responses, and 2 cases of stable disease. The objective response rate (ORR) was 54.55% (6/11), and the disease control rate (DCR) was 72.73% (8/11). Pathological results were available for 7 patients, among whom 2 achieved pCR (28.6%), and 1 achieved MPR (42.9%). Conclusions: Neoadjuvant adebrelimab combined with SOX and nab-paclitaxel has shown promising preliminary efficacy in patients with resectable LA-G/GEJ adenocarcinoma.
