Abstract

e22526 Background: The Colorectal Cancer Alliance developed a digital Screening Quiz and live navigation model to promote colorectal cancer (CRC) screening, education, and support. This risk stratification tool connects respondents to certified cancer care navigators, delivering evidence-based interventions to reduce barriers to care. The program utilizes mass media, social sharing, one-on-one education, and patient navigation to encourage on-time CRC screening, which has proven to improve outcomes. Methods: The Screening Quiz has been disseminated via social sharing, community outreach, and influencer campaigns to engage at-risk communities. The quiz assesses personal and family history, hereditary syndromes, and demographics to provide CRC screening recommendations based on USPSTF guidelines. The Alliance’s patient navigation team triages respondents overdue for screening. Symptomatic or high-risk individuals receive phone and email support, while average-risk individuals receive resources tailored to their insurance status. At-risk respondents are connected to financial assistance, free at-home screening tests, and colonoscopy scheduling services. Results: From July 2022 to December 2024, the Screening Quiz generated over 56,000 screening recommendations. Respondents had a median age of 46, with 32% aged 45-64. Most respondents were female (71%) and White (78.8%), though nearly half of those opting into navigation identified as BIPOC, including 17.3% Black/AA, 6.3% Asian, and 2.1% AI/AN. Hispanic/Latino ethnicity was reported by 8.7% of respondents. Symptomatic individuals accounted for 66.0% of respondents, while 6.5% were high-risk and asymptomatic. These groups represented 87.5% of the 2,139 individuals triaged to navigation services. Conclusions: The digital Screening Quiz, coupled with live navigation, is a promising model for increasing CRC screening rates and addressing care disparities. The Colorectal Cancer Alliance effectively engages diverse populations through digital tools, personalized interventions, and national partnerships. Future efforts will focus on leveraging relationships with healthcare providers and community organizations to expand outreach and close critical screening gaps.
